ACM case management services are designed to help you achieve and maintain a successful life within your community. That means helping you find local resources to meet your needs, from food and shelter to advocacy efforts. Case managers can also help you manage your finances, navigate paperwork, plan for emergencies, and obtain benefits.
Our case managers can provide these services and more to improve your life:

  • Connecting to support groups
  • Self-care and coping skill education

All of ACM case managers are Qualified provider and can help you overcome the environmental barriers that you encounter.

Outpatient Treatment Services

Access Care Management (ACM) Services is a community mental health and substance use disorder treatment program and provider of the King County Integrated Care Network (KC-ICN).

Our program focuses on serving adults with mental illness and/or substance use disorders to help improve individuals’ quality of life. Range of services includes diagnostic assessments, individual therapy, peer support, case management, group counseling, skills training, care coordination, medication management, and psychoeducation.

ACM clinicians provide services in a variety of locations across County, including within Permanent Supportive Housing programs, shelters, day centers. Services offered utilize Harm Reduction and Recovery principles, with an emphasis on building collaborative relationships with the persons served as the first step towards lasting engagement in treatment.

Homeless Outreach Services

In partnership with multiple agencies across the region, the Homeless Outreach team provides support to homeless adults in County and to Families in Shelter throughout the County.

Outreach Specialists offer community-based outreach and engagement services to people who are living in homelessness, primarily in the county. Outreach Specialists will meet people literally where they are at and assist with identifying needed resources and make linkages for housing, health care, and benefits wherever possible.

Services are prioritized for people who have had frequent inpatient hospitalizations and/or incarcerations over the last year. Staff work to identify the needs of individuals and provide information and referrals to needed services such as housing, inpatient substance use treatment, and long-term behavioral health counseling.
