Pediatric Complex Care Management

At ACM, we understand that keeping children well is as important as treating them when they’re sick. Our Pediatric CCM is a comprehensive approach aimed at providing optimal care for children with complex medical needs. This multifaceted strategy brings together a team of healthcare experts, including pediatricians, specialists, nurses, therapists, and social workers, to collaboratively develop and execute tailored care plans.

Pediatric care coordination plays a pivotal role in ensuring comprehensive and effective healthcare for children. It serves as the critical link that connects various healthcare providers, specialists, therapists, and support services involved in a child's care journey. By facilitating seamless communication and collaboration among these stakeholders, pediatric care coordination prevents fragmented and disjointed care that can negatively impact a child's health outcomes. This approach is particularly vital for children with complex medical needs, as their conditions often involve multiple organ systems and require a holistic understanding of their overall health. Pediatric care coordination ensures that medical information, treatment plans, and interventions are shared among the care team, allowing for timely adjustments, early detection of issues, and a unified approach to addressing the child's health challenges. 9 Months	Physical exam, growth and development, finger stick (hemoglobin), vaccines (if needed)
12 Months	Physical exam, growth and development, TB skin test (if needed), vaccines (if needed)

4 Years and Older	Physical exam, growth and development, vaccines (if needed) Annual check-up recommended, and if needed may include finger stick (hemoglobin), urinalysis, and/or vision/hearing screening.
